Persons or companies who wish to join DAMA-MN can submit membership applications and pay fees in several ways:

Membership Applications

• Download MS-Word or .PDF form and mail it in
• Download MS-Word or .PDF form and submit via e-mail.

Pay Membership Fee

• Make an On-line Payment
• Include Payment with mail-in form

 
All applicants will receive a receipt upon payment of membership fees.

Companies that require a bill including a purchase order (PO) number can submit the PO electronically as an upload or mail a copy with their application. If only the PO Number is required on the bill please include it with your electronic or paper submission.
